Hypertension Resources For Health Professionals


Hypertension management is a key priority for BLMK ICB. CVD Prevent data indicates that within BLMK there 64,000 people with undiagnosed hypertension and an additional 50,000 people with known uncontrolled hypertension, approximately 45% of the hypertensive population. Appropriate management through improved diagnosis and ensuring those with an existing diagnosis are treated to their target blood pressure could prevent 160 heart attacks and 240 strokes over three years.

Inform patient regarding New Medicines Service
Consider informing patient regarding NMS service offered by community pharmacies if patient has been started on new therapy to manage hypertension.

Community pharmacy hypertension case finding
The NHS Community Pharmacy Blood Pressure Check Service supports risk identification and prevention of cardiovascular disease (CVD).

A community pharmacist will opportunistically measure the blood pressure of consenting adults who come into the pharmacy, by offering anyone a free blood pressure check, followed by Ambulatory Blood Pressure Monitoring (ABPM) if they have a raised BP, who :

  • appears to be over the age of 40;
  • has not previously been identified as having hypertension or a related condition; and
  • has not had their blood pressure measured by a health professional within the previous six months.

General practices can also refer patients to a participating community pharmacy for a clinic blood pressure reading or for 24-hour ambulatory blood pressure monitoring. If referred this can include people with a known diagnosis of hypertension.
